[17 CCR 2500 (h) (i)] - What are the Reportable Communicable Diseases?
Emergencies: Report IMMEDIATELY by telephone.
Monday - Friday 8 AM - 5 PM: (530) 225-5591

Otherwise, Report within 1 WORKING DAY of identification by phone, fax (530) 225-5074,
or mail to Shasta County Public Health, 2650 Breslauer Way, Redding, CA 96001.
All other Diseases/Conditions should be reported by phone, fax or mail within 7 calendar days of identification using the appropriate Confidential Morbidity Report form.
These forms can be filled out online, then faxed to the confidential fax number.

Confidential Morbidity Report - Form A. Use this form for reporting all conditions,
except HIV/AIDS, Tuberculosis, and conditions reportable to DMV. -
Confidential Morbidity Report - Form B. Only use this form for reporting Tuberculosis.

Confidential Morbidity Report - Form C. Use this form for reporting lapses of consciousness or control, Alzheimer's disease
or other conditions that may impair the ability to operate a motor vehicle safely (pursuant to Health and Safety Code Section 103900). -
To Report HIV/AIDS: Adult HIV/AIDS Confidential Case Report form.
Fax HIV/AIDS Adult Confidential Case Report (ACRF) to the Confidential HIV/AIDS fax line 530-225-3768. - To report an environmental or occupational public health hazard, visit the Environmental Health Division page.
California Reportable Disease Information Exchange (CalREDIE) Provider Portal
Healthcare Providers can report and submit all required Title 17 Communicable Disease CMR’s electronically
through the online California Reportable Disease Information Exchange (CalREDIE) Provider Portal.

The CalREDIE Provider Portal (PP) provides secure access for Healthcare Providers to electronically submit Confidential Morbidity Reports (CMRs), required by California Code of Regulations (CCR), Title 17, Section 2500 et seq., directly to Local Health Departments (LHD).

CMRs received via the PP are available in real-time to Local Health Staff for investigation and follow-up, thus reducing the burden of data entry at the local level and increasing the timeliness of reporting.

**Failure to report is a citable offense and subject to civil penalty ($250) (Health and Safety Code 105200).
